Petition to Appeal Rates Established by the Board of Directors

XYZ Water Supply Corporation

134 Main Street

Any town, Texas 99999

We, the customers of XYZ Water Supply Corporation, hereby appeal the decision of the board of directors to raise our rates. The new rates started on September 1, 2000, and apply to water utility service, tap fees, late fees, and reconnect fees. We were notified of the change on September 5, 2000.

We are asking the Public Utility Commission of Texas to review this decision and determine if the new rates are just and reasonable.

The old rate was a $20.00 base charge plus $1.25 for every 1,000 gallons used. The new rate is a $25.00 base charge plus $2.00 for every 1,000 gallons.

We have chosen the XYZ Property Owners Association to represent us. Please send any communication to Ms. Jane Doe, President, at 1235 Main Street, Anytown, Texas 9999, or call 512-555-4321.
